Four years ago on June 24, 2022, the Supreme Court of the United States issued what pro-life advocates called the most consequential ruling in a generation. In a 6-3 decision, the Court overturned Roe v. Wade in Dobbs v. Jackson Women’s Health Organization, ending nearly five decades of a federally imposed abortion regime that conservatives had long argued was built on a constitutional foundation that never existed.

The decision was authored by Justice Samuel Alito, who wrote bluntly that Roe was “egregiously wrong from the start.” Alito argued that the original 1973 ruling, which invented a right to abortion out of the due process clause of the Fourteenth Amendment, had not settled the abortion debate but had instead “inflamed” it and “deepened division” for half a century. With Dobbs, the Court sent the question of abortion back to the people and their elected representatives, where conservatives had always insisted it belonged.

The political earthquake that produced Dobbs was decades in the making. President Donald Trump, in his first term, appointed three justices to the Supreme Court whose confirmation votes came under fierce Democratic opposition.

Justices Neil Gorsuch, Brett Kavanaugh, and Amy Coney Barrett joined a majority that, along with Justices Clarence Thomas and Samuel Alito, formed the decisive six votes that reversed Roe. For the pro-life movement, it was the fulfillment of a 50-year legal and political campaign.

The immediate aftermath of the ruling saw states move swiftly. Within days and weeks of the decision, states that had long awaited this moment began enforcing existing laws or passing new ones. According to data tracking the legal landscape as of 2026, 21 states now ban or restrict abortion earlier in pregnancy than the standard Roe had set.

These laws largely exist in the South and heartland of America, reflecting the views of the voters who elected the legislatures that passed them.

The pro-life movement had reason for celebration in those first months. For the first time since 1973, the legal framework of abortion in America was no longer dictated by an unelected federal judiciary imposing a national standard. State legislatures in Mississippi, Texas, Alabama, and dozens of other states were finally free to pass laws reflecting their constituents’ values on human life.

Kristan Hawkins, president of Students for Life of America, was standing outside the Supreme Court when the opinion was handed down. She recalled reading the key line aloud: “The Constitution does not confer the right to abortion. Roe and Casey are overruled.” For Hawkins, it was the beginning of a new chapter, not the end of the fight.

“The decision of abortion is now returned to the people and their representatives,” she said. “We have 51 playing fields in our country where we’re attempting every single day to pass laws to either outright end abortion or to severely restrict abortion.”

But four years on, the anniversary carries a more complicated tone within the pro-life movement. While the legal victory at the Supreme Court remains intact, a growing crisis is threatening to undo much of the practical impact of state-level restrictions.

That crisis is the mail-order abortion pill industry, and it has become the dominant concern among pro-life leaders entering the fifth year after Dobbs.

According to data from the Guttmacher Institute, a pro-choice research organization whose numbers are widely cited, clinicians performed approximately 1,126,000 abortions in 2025, a figure that is up 16 percent from 2020, the year before the Dobbs litigation reached its peak.

That number does not include abortions performed at home using pills obtained through community networks, foreign online pharmacies, or other informal channels that exist largely outside the reach of state enforcement.

Bob Vander Plaats, president and CEO of The Family Leader, an Iowa-based evangelical political advocacy organization, delivered a stark assessment on a pre-anniversary press call. “Are we better off today? No, we’re killing more babies today,” he said. “We’re being more creative about how we kill more babies. We’re endangering more women today.” It was a sobering message from a movement that spent decades working toward the Dobbs outcome.

The root of the problem, according to pro-life leaders, traces back to the Biden administration. In 2023, the Food and Drug Administration, under Biden-era direction, eliminated the in-person prescribing requirement for mifepristone, one of the two drugs in the standard abortion pill protocol, and allowed the drug to be delivered by mail. That single regulatory change made state-level abortion restrictions functionally unenforceable for a large segment of the population who could simply order pills online and self-administer an abortion at home.

Bishop Daniel E. Thomas of Toledo, chairman of the U.S. Conference of Catholic Bishops’ Committee on Pro-Life Activities, released a statement on the anniversary warning that the victory of Dobbs “risks being undone by the massive influx of abortion pills.” He praised the ruling itself as a historic turning point but called on Catholics and pro-life leaders to remain engaged in the fight rather than treating Dobbs as the final word. “While the Dobbs decision gave states the freedom to pass pro-life laws and protect pre-born children, these laws are now being undermined,” he said.

The tension between the Trump administration and the pro-life movement on this front has grown more visible in 2026. Trump’s justices delivered the Dobbs decision, and the president has received credit from the movement for speeches, judicial appointments, and other actions over the years. But with midterm elections approaching in November 2026, the administration has grown notably quiet on further abortion restrictions, caught between its pro-life base and the coalition of independent voters who support abortion access and who helped deliver the 2024 election.

Marjorie Dannenfelser, president of Susan B. Anthony Pro-Life America, said on the same pre-anniversary call that the “states-only experiment is a failure.” Her comments reflected a broader frustration within a segment of the pro-life community that believes the battle must eventually move to the federal level if abortion is to be seriously restricted across the country. That position, however, runs into the political reality that Congress does not currently have the votes for a federal restriction, and that the issue remains a liability at the ballot box.

Pro-life leaders have also pushed back against what they characterize as an avalanche of deceptive state ballot initiatives since Dobbs. Ryan Bomberger of the Radiance Foundation said the movement faced “hugely deceptive state ballot initiatives” ensuring virtually unlimited abortion access in multiple states following the ruling. In the years since Dobbs, 16 states have held referendums on abortion rights, with outcomes largely going in the direction of expanded access in states where such measures reached the ballot.

On the constructive side, pro-life leaders point to significant investments in alternatives to abortion. Bomberger noted that 19 states now offer alternatives to abortion, including more than $200 million in pregnancy and post-partum resources, adoption options, counseling, and material support. “Funding for pregnancy centers has increased significantly,” he said, framing the movement’s work as a long-term cultural and material effort to support women and families, not merely a legal campaign.

The National Abortion Federation chose the fourth anniversary of Dobbs to release what it described as a new policy position openly calling for the elimination of all restrictions on abortion, including gestational limits that abortion advocates had previously treated as reasonable compromises. The move was criticized by pro-life observers as revealing the true agenda of the abortion industry, one that goes far beyond the exceptions and moderate language that appears in public polling.

Meanwhile, prosecutions of anti-abortion activists have continued to be a point of controversy. The Trump administration has taken steps to reverse Biden-era enforcement of the Freedom of Access to Clinic Entrances Act, and the DOJ’s posture toward such cases has shifted significantly since 2025. The pro-life community has welcomed that shift, arguing that the FACE Act had been wielded selectively against peaceful protesters.

For millions of Americans who identify as pro-life, the fourth anniversary of Dobbs remains a day of genuine thanksgiving. The constitutional distortion that had stood since 1973 was corrected by a Court willing to follow the text and history of the Constitution rather than the preferences of an elite consensus. The return of the question to democratic governance, however imperfect and contested the results, represents the proper constitutional order.
